Complete the Exit Survey (REQUIRED)
- Students Name
- Date of the Session
- Participation/Engagement Questions (REQUIRED)
- Comments that are sent to the Student (not required but appreciated)
- Check the box if you would like admin to review the survey and or the session
- Comments that are sent to Admin (not required but appreciated)
- Comments that are for yourself (if desired)
- Save the completed survey

Pearl Post-Session Surveys are required after every tutoring session and must be completed in their entirety. These surveys serve as the student’s official progress notes and are used to document academic growth and progress.
Please ensure your responses are thorough, specific, and informative. Avoid vague or one-word answers, and make sure each section is completed accurately based on that day’s session.
Although tutoring sessions are expected to last for the full scheduled session time, if a session must end early for any reason, you MUST provide an explanation in the “Notes to Admin” section of the survey detailing why the session was cut short.

Reporting State Standards in PA Cyber Session Surveys
Step 1: Open the PDE SAS Standards Website
Copy this URL from your session survey note:
https://www.pdesas.org/Standard/ViewOpen a new browser tab and paste the link to access the Standards page.
Step 2: Filter by Student Grade and Subject (Grades K–8)
On the left-hand sidebar, select your student’s Grade (e.g., 3rd Grade).
Scroll down to the Subject section on the left sidebar and check the applicable subject box (e.g., Mathematics).
Step 3: High School Additional Filtering (Grades 9–12)
For High School Students only: After selecting the grade and general subject, check the Course section on the lower left-hand sidebar (e.g., Algebra I, Literature, Biology). High school standards are often listed directly under specific course titles.
Step 4: Copy and Paste the Standard
Review the generated list on the right side of the screen to find the standard covered during your session.
Highlight and copy the full text, including the standard code (e.g.,
Standard – CC.2.3.3.A.1Identify, compare, and classify shapes and their attributes…).Switch back to your PA Cyber Session Survey tab and paste the text into the designated survey answer box.
