Waiver of Liability for tutoring during COVID-19
Student and Family Form
At-Home Tutoring Services is implementing the below protocol for tutors and students for in-person delivery of educational sessions:
• Prior to arrival, Tutor and Student must take their temperatures. If fever is detected, the session will be canceled;
• Each Student’s family member, Student and Tutor must wear facial protection and gloves during the entire scheduled duration of the tutoring session while in the designated tutoring locale where services are to be delivered;
• Each Student’s family member, Student and Tutor must wash hands with soap and water for at least 20 seconds, immediately upon Tutor’s arrival to the designated tutoring locale, after going to the bathroom, before eating or after blowing nose, coughing, sneezing, etc.
• Tutor will refrain from touching anything unnecessary in the Student’s residence and will refrain from going into any area of the house other than the designated tutoring area;
• Student or his family will wipe and disinfect any surface areas in the designated tutoring area before each scheduled session;
• Tutor will bring own cleaning materials and will wipe and disinfect any surface areas in the designated tutoring area before each scheduled session as an additional precaution;
and
• Tutor and Student shall remain 6 feet apart in distance during the tutoring session when practical.

We appreciate the opportunity to deliver in-person, high-quality education during these challenging times and understand the value of in-person sessions over remote and virtual sessions for some students. We thank you for your cooperation in adhering to these guidelines to ensure the safety of our clients, client’s family and tutors.
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID- 19 by having a tutor from At-Home Tutoring Services deliver services and that such exposure or infection may result in personal injury, illness, permanent disability, and death.
I understand that the risk of becoming exposed to or infected by COVID-19 from an At-Home Tutoring Services’ tutor delivering in-person tutoring sessions may result from the actions, omissions, or negligence of myself and others, including, but not limited to the Student, Student’s family or Tutor.
I voluntarily agree to assume all the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with At-Home Tutoring Services’ tutor delivering in-person tutoring sessions ("Claims").
On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless At-Home Tutoring Services, its employees, agents, representatives, and its independent contractors of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the At-Home Tutoring Services, its employees, agents, and representatives, its independent contractors, whether a COVID-19 infection occurs before, during, or after participation in any At-Home Tutoring Services’ tutoring program.
